Registration Form for Beneficiary Designation
You may name a beneficiary or change your beneficiary designation by completing this form for Beneficiary Designation, and returning it to the Pension Fund Office.
Ten Year Certain Beneficiary Designation Change Form
If you are currently receiving retirement benefits with the Ten Year Certain option, and wish to change your beneficiary, please complete this form, and return it to the Pension Fund Office.
Please Note: If you die without naming a beneficiary, any benefits payable will be paid to the first of those who survive you in the following order: your spouse; your surviving children and descendants of your deceased children; your parents; your brothers and sisters. If none of these survive you, any benefits payable will be paid to the administrator or executor of your estate, or according to Wisconsin statutes, at the discretion of the Trustees.
If your spouse is named as your Beneficiary, and you divorce, the beneficiary designation is cancelled on the date of divorce. If you want your ex-spouse to remain as your beneficiary, a new Pension Fund Registration Form for Beneficiary Designation must be completed after the divorce to re-designate your ex-spouse as your beneficiary.
Reciprocity Transfer Request Form
If you worked in this Fund’s vicinity and had hours reported here but wish to have them transferred to your home fund, fill out this form and return it to the Pension Fund Office.
Reciprocity Stop Transfer Request Form
If we are currently transferring your pension contributions to another pension fund, and you would like to stop that transfer, fill out this form and return it to the Pension Fund Office.

Federal Income Tax Withholding Form 2025
To change your Federal Income Tax Withholding or have Federal Income Tax withheld from your monthly benefit, please complete this form and return it to the Pension Fund Office.
Wisconsin Income Tax Withholding Form
To change your State of Wisconsin Income Tax Withholding or have Wisconsin Income Tax withheld from your monthly benefit, please complete this form and return it to the Pension Fund Office.
Automatic Joint Survivor Waiver Form for Married Participants
If you are married, and you select an option OTHER THAN the Automatic Joint & Survivor option, this form must be completed by both you and your spouse. Your signatures must be witnessed by a Pension Fund Benefit Representative, or notarized, if signed elsewhere.
Declaration and Waiver Form for Unmarried or Separated Participants
If you are single, the left side of this form must be completed. If you are single and do not select the Lifetime Only option, the right side must also be completed. Your signatures must be witnessed by a Pension Fund Benefit Representative, or notarized, if signed elsewhere.

2021 Summary Plan Description
The Summary Plan Description (SPD) contains a detailed summary of the rules and regulations of the Plan. It describes what benefits the Plan provides and how the Plan works.
Summary of Material Modifications (SMM)
Changes made to the Plan since the latest SPD was written are included in the Summary of Material Modifications.
Military Service Notice
If you served your country in the U.S. Armed Forces or National Guard after June 1, 1959, you may be entitled to pension credit during the time you were in the Armed Forces.
